First steps
The first thing you need to do if the cap is crumbling is to clean out the old mortar using a ball peen hammer and a chisel. If you have not done this before make sure you have goggles and tools for safety. It may take a while to chip away the old mortar but when finished make sure you brush any loses debris away.
Once you're happy with this its time to apply a new layer of ready mixed mortar, the idea here is to slop it away from the flue so rain can flow and drain off properly.
If you need to repair the mortar around the bricks you'll need to follow the tips and remove the old mortar using a ball-peen hammer and a cold chisel. Apply to new mortar with care and package the cracks you should use a weather resistant ready mixed mortar.
